A support service for older and/or vulnerable people
with independent living at home
Gosport Borough Council and Fareham Borough Council are working together in a joint pilot project called Telecare that has been funded by the Preventative Technology Grant (PTG).

Telecare is a service that enables people, especially older and more vulnerable individuals, to live independently in their own home. Of those asked if they would prefer to stay in their own homes or move to receive support, 90% say that that is their wish to stay at home and Telecare allows it to happen. Equally it gives peace of mind to family, friends and carer’s, knowing that when they are unable to watch over their loved ones, Telecare will.
The Telecare project utilises a range of technology products and services that are designed to improve quality of life; giving the user the confidence and ability to remain at home, safe in the knowledge that help will always be available when and if it is needed. By reducing the burden for relatives and carer’s, Telecare technology empowers people to manage their own health and well-being and to live a more independent life.
Telecare also enables discharge from hospital and reduces pressure on acute services.
This type of support service is particularly beneficial to those individuals who do not have sufficient care or support at home or from nearby relatives.
As part of this service, we can provide a 24/7 response cover, however not all clients who have a Telecare system in place will want a response service; favouring their own family or friends instead. This choice will be made at the referral stage but can be re-assessed if circumstances change. Telecare is primarily aimed at those individuals who have either no family or family that do not live local to them.
For the pilot period we will work in partnership with Chichester Community Careline who has an excellent Telecare service already set up. By working in partnership with them we hope to achieve not only a successful working relationship, but also to provide the same high standard of service in both Gosport and Fareham Boroughs.
Telecare involves placing a variety of discreet sensors around the home, to monitor for personal risks and a full range of needs likely to be experienced by the client group (e.g. dementia, vulnerability to falling, epilepsy etc). The innovative range of non-intrusive Telecare sensors, work with Lifeline home units to offer a comprehensive way of managing the risks to a person’s health and home environment, 24 hours a day, 7 days a week.
The Lifeline 400 is designed to protect people living in their own homes. Supplied as standard with either an Amie+ or Gem+ personal radio trigger; the Lifeline 400 can be used to raise an alarm call from anywhere in the home by simply pressing the radio trigger, or the large illuminated red button on the unit. Calls are received at the dedicated 24 hour response centre, where the appropriate action can be taken.
