Title
The title explains what the form allows you to do.
Introductory Text
The introductory text explains what you need to do to complete the form.
Fields
Fields are where you enter information into the form. Each field has a label to the left which tells you what to enter into that field. Fields work in different ways; you may need to enter text or numbers into a box, select an entry in a drop-down box or list, tick a check-box or select a button.
Navigation Buttons
The '< Back' and 'Next >' buttons allow you to move between different pages in the form. Once you have completed a page, click 'Next >' to continue on to the next page. If the 'Next >' button is disabled this is because you have not completed the required fields on that page (see status bar). On the final page of the form the 'Next >' button is replaced by a 'Submit' button. Press the 'Submit' button to send the form.
Reset Button
This button allows you to clear any fields you have completed and takes you back to the start of the form.
Status Bar
The Status Bar gives instructions on which fields you need to complete and what to do if you have entered incorrect information into any fields. Older computers may display the form in a different way without the status bar, in this case if you make a mistake the form will provide instructions underneath that question on how to complete it correctly.
Progress Bar
The Progress Bar is used on longer forms to show how much of the form you have completed.
Padlock
The padlock icon tells you that the information in the form will be sent encrypted (closed padlock) or un-encrypted (open padlock). Encryption is the high level of security that computers use to send information safely across the internet. It makes the details you send unreadable to people who should not see them.
